Credentials: High School diploma or GED with one year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder. Must have valid driver’s license with 2 years of driving experience.
Special skills: Conscientious, Dependable, Good computer skills, Works well with other staff, Good communication skills.
Primary functions:
Teaching clients the following skills: how to utilize/access community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
Complete documentation in EMR within 24 hours of service delivery.
Conduct activities necessary to allow individuals to live with maximum independence in the community.
Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.
Maintain all trainings and required supervision.
Funding Source: Meet requirements 908 KAR 2250, Billable for H2015, Medicaid.
