Degree/Credentials: Bachelor’s degree in health and human services or related field.
Special Skills: Ability to maintain effective, professional, and positive relationships with staff, volunteers, and program participants is required. Must have a valid driver’s license and vehicle. Must be able to work independently and have excellent management skills.
Primary Functions:
Conduct a wrap-around assessment to identify all barriers each participant faces and develop a solution for them. Facilitate short-term educational groups (i.e. Relapse prevention, life skills) and provide linkage to referral and community resources.
Transporting program participants in personal vehicle will be required. Attend and participate in training activities and meetings is required.
Perform any other duties as assigned.
Shift Days/Hours: to be determined; must be willing and able to work some evenings and weekends.
