Title: Service Coordinator

Company/Location: Kentucky River Community Care/Can be based in either Breathitt, Lee, Owsley or Wolfe Counties

Employment type: Regular Full Time

Credentials Required: Bachelor's degree in approved Human Services field (Social Work, Psychology, Sociology, etc.) with one (1) year post graduate work with children.

Special Skills/knowledge: good communication skills, maintain client related goal data, including case notes, outreach, and relationships with community partners.

Primary Functions:

Provide case management services including needs assessment, service plan and monthly contacts.
Develop and coordinate community wide outreach for youth.
Develop plan of intervention that address needs for housing, services, crisis and intervention.
Link clients to available resources to assist with needs and progress; assist client with resolving identified barriers; monitor clients' needs and progress, coordinate and facilitate with treatment team to assist client with reaching person centered goals.
Network with agencies, coalitions, and local community meetings.

Shift/Days and Work Hours: Monday-Friday, 8:00am - 4:30pm

Salary: $17.50 hour
